may reinstate your policy within three (3) years from the
date of lapsation by submitting the following requirements
to any of our offices nearest you:
A
properly completed (i.e.
must be dated, signed and witnessed) Health
Statement Form
Payment
of reinstatement cost
You
will be advised if other requirements such
as a medical examination is necessary.
