may submit a properly completed (i.e. must
be dated, signed and witnessed) Request
for Policy Change Form
to any of our offices nearest you.
Photocopy
of your marriage contract if the change of name is due
to marriage.
Copy
of the Finality of the Decision of the Court if the
change is due to legal separation.
Photocopy
of any valid proof of your correct name
(Birth Certificate, Baptismal Certificate,
Passport ID, Driver’s License, Valid
company ID, SSS) if you want to correct
your name due to the error in filing.
