offering wide amounts of coverage at quite reasonable premiums.
- Consists
of the GYRT, pre-packaged with the AD&D and TPDIB riders.
- Coverage
amounts range from P100,000.00 to P500,000 in multiples
of P100,000.00.
- Payment
mode is annual only.
- Eligible
groups are employee-employer groups of standard occupational
risk only.
- Eligible
insureds are regular, full-time, actively at work employees,
ages 18-64.
- Minimum
of 5 lives up to 25 lives.
- Participation
requirements if the plan is company paid; 75% participation
if the plan is paid by the employer and employee.
- Insureds
ages 50 and over shall be required to fill up a health statement.
