A. Who can join

• All policyowners with inforce premium paying policies.

• Policyowners who responded to company initiated campaigns.

B. How to qualify

1. For the Quarterly Draws

• One (1) raffle entry shall be given for every P500 single premium payment made on or before the due date.

• Five (5) raffle entries will be given for existing policyowners who respond to a company campaign or offer, within the promo period (January 1, 2006 to December 28, 2006)

• Double raffle entries shall be given for premium payments made on or before the due date following the policy anniversary of any policy, including Personal Accident Insurance.
  

2. For the Grand Draw

• Premium payments made after the due date but within the 31-day grace period shall be included in the Grand Draw only.

• All entries in the quarterly draws shall also be included in the Grand Draw.
  

*All payments must be made at any Philamlife office or payment facility accredited by Philamlife. Premiums paid from automatic premium loans, dividends and waived premiums due to disability are not included.

C. Prizes to be won

• Grand Draw - One (1) winner of P1,000,000

• Quarterly Draws - Each quarter, there will be five (5) winning policyowners of P100,000 cash each and P100,000 worth of Personal Accident Insurance coverage
  

D. Raffle Guidelines

1. Policy numbers of paid premiums and responses to company campaigns will serve as raffle numbers.

2. A computerized program will randomly select the winner.

3. Raffle entries earned from premium payments and responses to company campaigns shall be included in the Quarterly Draw corresponding to the inclusive dates below.

4. Policies of winning policyowners must be inforce at the time of the raffle date to qualify for the prize.

5. Policyowners qualified in the Quarterly Draws will still be included in the Grand Draw, provided their policies remain inforce at the time of the Grand Draw.

6. Policyowners who did not qualify in the Quarterly Draws may still qualify for the Grand Draw provided their premiums payments were made within the 31day grace period and their policies remain inforce at the time of the Grand Draw.

7. Policyowner may only win once during the same raffle draw. In the event that his/her name is drawn twice, he/she will get the higher prize.

8. Philamlife shall shoulder 20% local government tax for the cash prizes.

9. All draws will be held in public and witnessed by Philamlife representatives.

10. Winners will be notified through registered mail.

11. Winners should present Official Receipt, policy contract and proper identification documents when claiming their prize.

12. Prizes must be claimed within 60 days upon receipt of notification, otherwise they shall be forfeited.

13. Philamlife reserves the right to publish the photographs and/or names of winners in promotional materials and advertisements as it sees fit.

14. Employees of Philamlife and their relatives up o the 2nd degree of consanguinity or affinity are excluded from the raffle.
